Please complete this form to request Sober Monitor Training for your chapter. Please note that requests are fulfilled on a first come, first serve basis, based on availability of Psi Society. Your training is not confirmed until you receive an email from Psi Society confirming time, date, and location. Additionally, any cancellations or changes for trainings must be made more than 48 hours in advance. Last minute cancellation or failure to attend Sober Monitor Training may result in the inability to request the program for the rest of the semester. As a reminder, Sober Monitor Training is required semesterly for your newest members to serve as Sober Monitors!
