Requesting OSFL NETID Sponsorship
Complete the form below and you will receive your individual OIT Sponsorship Request Form from a staff member in the office. Due to the sensitive personal information that is included, we will be sending this information to you through a secure messaging system called the Vault. The Vault allows for protected information to be sent internally and externally to UT. Those of you using your UTK email address for communication will only need to log-in to receive the message. Those using Gmail, etc. will need to register that email address in order to receive the message, but that process is pretty simple and straightforward using the following website – https://vault.utk.edu.
Please note: After you have login, you will be able to view reports and documents. The Vault only keeps information for 20 days, so if you do not check the message and save the report when you receive it, it will delete and you will no longer be able to access it.
